International designed these engines to run on high pressure oil to hydraulically actuate each injector electronically by the on board computer which is separate from the fuel system which runs at 75 psi. This engine sounds like it's running on one cylinder but in reality one cylinder is dead with way too much fuel entering the cylinder.
To start these diesel engines they need 75 psi fuel pressure and psi injection control pressure supplied by the high pressure oil pump. At first it was a mystery so we had to decide what to check first. The only logical way to find this out was to pull all of the injectors for inspection.
The 6 injector turned out to be faulty only because of a failed seal washer which prevents compression to enter the injector sleeve in the cylinder head. The high pressure and heat proved too much for the o-ring seal on the injector body which keeps the fuel supply contained. Once the o-ring gave way compression entered the fuel supply system to the injector and aeration went into effect.
This causes a severe miss since air compresses and proper injection comes to a dead stop. Even worse every time the engine is shut down fuel would run into the cylinder sitting on top of the piston until the next start up smoking out the exhaust and washing away oil lubrication on the cylinder walls. This was a concern and now that we knew what the cause was it was time to think of a solution so it didn't happen again.

The sealing washer slides over the tip of the injector and is supposed to seal compression from entering the injector cavity. I would probly get my scope on the wire leads to the ipr and want to see, is the ecm trying to control ipr or not. Or install a light with the same ohm value at the ipr and see if it light up again is the computer trying to control the ipr. Below I pasted the first part of low icp IH flow chart. I have never actually read it over all the years. One interesting thing is on list of possible causes they talk about pressure sensors for boost possibly causing a problem. Again the jumping of the ipr would verify its a electrical issue weather its a wiring issue, input issue to ecm or ecm issue and not a mechanical issue.

All the codes except forare ICP related. This is usually a bad sensor. High pressure leaks will usually cause hard or no start when hot, but the engine will run ok once it is started. The valve cover gasket and harness is the same piece. We usually change them if the engine has a lot of hours on it. During that test the ecm is watching the air flow through the EGR valve by what the Exhaust back pressure sensor is reading. The test runs with the valve closed, then opens it and the ecm is expecting to see a change in the ebp, but doesn't.
The ebp sensor could be bad, the sensing tube could be plugged, or the egr valve not working. The ICP sensor is in the same location, under the valve cover, screwed into the high pressure oil manifold.

These high pressures create problems if there is a leak in the high pressure manifold or anything else related in the system. The HPOP high pressure oil pump supplies the high pressure engine oil to the hydraulic operated injectors so if pressure drops the injection system will fail or run poorly.
Our shop has had some experience with low ICP which can be mechanical due to a faulty o-ring on an injector or high pressure manifold. One step for troubleshooting is applying shop air to the high pressure manifold which eliminates the high pressure oil pump all together. Shop air usually runs around psi so when that is hooked to the manifold inlet the leak will be obvious most times.
An ICP pressure sensor code will fool the engine computer with an incorrect reading which will derate the engine. Working with high pressures causes problems and seeks out all of the weak spots. With emissions the goal is to create high pressure fuel injection so combustion is cleaner reducing carbon and soot.
We are comfortable with the troubleshooting process on these systems but it takes time. The steps to get to the parts on the injection system adds down time due to the difficulty of removing the valve cover and high pressure manifold. Most emission controlled diesels have an array of harnesses and lines in the way when it comes to disassembling the top end. Two to three days is not uncommon for repairs to be completed. The cost could be excessive if injectors or sensor replacement is required.
We are constantly dealing with engine codes when ICP pressures are not within the parameters. The one advantage for us is we have dealt with all associated issues since they tend to repeat themselves.
